JENNIFER MARTINEZ, MPH
Jennifer's work has focused on programmatic implementation of Health Information Exchange and the development of innovative initiatives to encourage and incentivize systems improvement within local health care safety net systems. Those projects have included improving access to care, care transitions, integration between behavioral health and primary care, and integration of data systems – all critical pieces of delivering whole person care. Jennifer was the key leader in Alameda County's Community Health Record and Social Health Information Exchange creation and implementation - from concept phase to full implementation over a 6-year period.
BRIDGET NOLAN SATCHWELL
Bridget specializes in turning complex policy and program concepts into concrete, operational systems. She brings more than 20 years of experience in program design, operations, and implementation for nonprofit and government organizations, with her most recent work focused on consulting with organizations navigating California’s Medi-Cal transformation under CalAIM.
Her work includes hands-on operational and implementation support in the CalAIM space, with particular depth in Housing-Related Community Supports and Enhanced Care Management (ECM). She previously supported Alameda County’s multi-year Whole Person Care implementation and its transition to CalAIM, giving her a strong, practical understanding of how large-scale reforms move from policy to practice. Her consulting experience also includes Early Childhood initiatives and the implementation of Social Health Information Exchange (SHIE) efforts. From international settings to local systems in the Bay Area, her work reflects a consistent focus on making ambitious ideas operational, sustainable, and meaningful for the communities they are designed to serve.
