Fully-implemented results – We shepherd concepts from theory to practice, ensuring systems are fully operationalized.

Robust measurement – We qualitatively and quantitatively assess all points of view to measure progress, impact, and outcomes.

Consensus and engagement – We use clear communication, data visualization, and plain language to orchestrate consensus across sectors, establishing a foundation for engagement.

How we do it:

System integration – We align systems and connect sectors so people can thrive.

Program design – We lead discovery, identify stakeholders and resources, design interventions, and evaluate, ensuring programs are consistent with applicable regulations, impactful and sustainable.

Strategic planning – We craft plans, not just concepts. Our products can be implemented, disseminated, measured, and adapted for other use cases.

Partner relations management – We help identify who needs to be at the table and what matters to them so all partners have a stake and stay engaged.

JENNIFER MARTINEZ, MPH

Jennifer's work has focused on programmatic implementation of Health Information Exchange and the development of innovative initiatives to encourage and incentivize systems improvement within local health care safety net systems. Those projects have included improving access to care, care transitions, integration between behavioral health and primary care, and integration of data systems – all critical pieces of delivering whole person care. Jennifer was the key leader in Alameda County's Community Health Record and Social Health Information Exchange creation and implementation - from concept phase to full implementation over a 6-year period.

BRIDGET NOLAN SATCHWELL

Bridget specializes in managing the process of concepts becoming concrete products, actions, or programs. Her work includes program, operations, and implementation work for non-profit and government entities. Recent experience includes doing this work as part of Alameda County's 6-year $190 million Whole Person Care Medi-Cal Waiver implementation and supporting their transition from Whole Person Care to CalAIM. Bridget has been following her passion for underserved communities throughout her 15+ year career in non-profit (internationally and in the US) and local government. Whether in eastern DR Congo, the San Francisco Bay Area, or somewhere in between, Bridget’s work has focused on new program start-ups and major program transitions. From HIV treatment programs to 9-1-1 dispatched behavioral health teams that are an alternative to police response, taking great ideas, making them real, and watching them positively impact communities has been the consistent theme and highlight of her work.
